During a Council meeting held virtually on Thursday, 21 October 2021, Ceredigion Councillors approved the Through Age and Wellbeing Strategy for 2021-2027and action plan for Ceredigion.

The strategy sets out the vision and associated approaches to transform how people’s health, wellbeing and safety can be supported in Ceredigion.

It aims to identify at an early stage what concerns people have and aim to prevent escalation, wherever possible, through a timely and proportionate response.

The strategy also provides a model to deal with the root causes of concerns and includes key objectives and focused areas of need. The strategy and action plan progresses the initial work to restructure services in 2019 which was slightly delayed in 2020 due to the pandemic.

However, the transformation of services has now progressed well, social care and lifelong learning have integrated into three services – Porth Cymorth Cynnar, Porth Gofal and Porth Cynnal. These services, along with Customer Contact, make up the four main areas that fall within the Through Age and Wellbeing Programme.

Councillor Alun Williams, Ceredigion County Council Cabinet Member for Porth Cynnal – Specialist Through Age Services, said: “The purpose of the Strategy and Action Plan is to outline what is required over the next three years and beyond to address the root causes of problems experienced by vulnerable people in our communities and then demonstrate how we plan to address them. We believe it is essential to work closely alongside our local partners in all the different groups and organisations across Ceredigion to help rebalance the care and support available and provide sustainable services within the county. The root causes of difficulties can include mental and physical ill-health, substance misuse, financial concerns, domestic abuse, poor housing, dementia and more. Families, carers and support networks are an essential part of the local well-being picture. This new model fully recognises this and is designed to ensure people get the right level and type of help early enough to prevent, reduce or delay the need for ongoing support, and to maximise people’s independence so that they can remain in their own home and community for as long as possible.”

Councillor Catherine Hughes, Ceredigion County Council Cabinet Member for Porth Gofal, Early Intervention, Wellbeing Hubs and Culture, said: “It’s a great step forward to see the Strategy and Action Plan for the Through Age and Wellbeing being implemented. Our vision is to ensure every child, young person and adult in Ceredigion can reach their full potential. We want to ensure fair access for all to excellent universal and targeted services that support the health and wellbeing of all citizens. It includes the key development of Wellbeing Centres across the County that will integrate with our leisure services as well as providing mobile or pop-up provision in surrounding towns and villages. We will work with our partners to offer a range of support and intervention in order to improve the health and wellbeing of all our communities. This includes developing skills and resilience that will last a lifetime and enable individuals to cope well with the challenges and pressures that they may face.”

The Through Age and Wellbeing Strategy will ensure that the requirements of the Social Services and Wellbeing (Wales) Act 2014 are being achieved and supports the requirements of the Wellbeing of Future Generations (Wales) Act 2015.
