Service provided	The Assessment & Support Team undertakes assessment and care planning and the provision and review of community based services for children, young people and their families throughout Ceredigion. In practice, this means that the team provides: an initial response to new referrals a time limited service to children, who are looked after by the authority, a service to children whose names are on the Child Protection Register, services to children in need including disabled children.

People referring in their professional capacity will also need to complete and submit an Interagency Referral and initial information form (see link at top of page)
